    DFA Passport Requirements for First-Time Minor Applicants

    Here are the DFA requirements for first-time minors:

    • completed application form
    • Confirm Online Appointment(If it's true)- Children under the age of 7 do not need to make an online reservation. They have the right to use service routes with their parents and minor siblings.
    • Personal images of the minor applicant and parent or authorized adult companion
    • Philippine Statistics Authority (PSA) birth certificate.
    • If the PSA birth certificate is unclear or illegible, a copy from the local registry may be required.
    • If the child is accompanied by only one parent, a marriage certificate is required.
    • If the parents are unmarried, a statement of support and/or consent from the mother is required if the mother is not accompanying the child.
    • If the affidavit of support and/or consent was issued abroad, it must be notarized by a Philippine embassy or consulate.

    Additional Requirements for Special Circumstances (Minor)

    For special cases such as late registration, absence of birth certificate, illegitimate child status, dual citizenship or naturalized Filipino, additional supporting documents must be submitted.

    Applicants who are not accompanied by their parents during the application process

    • Applicants must submit a special power of attorney issued by a parent designating an accompanying minor to assist with the application. Special Power of Attorney if issued abroad, must be notarized by the Philippine Embassy/Consulate.
    • Passport or valid ID of authorized adult companion
    • A special power of attorney or alimony and consent must be accompanied by a copy of the parents' valid ID and/or passport.

    Applicants do not travel with their parents.

    • Applicants must submit a DSWD permit and an affidavit of support and consent issued by their parents. If executed abroad, the Affidavit of Support and Consent must be consularly notarized by the Philippine Embassy/Consulate.
    • Passport or valid ID of the authorized adult companion.

    The applicant is an illegitimate child under the custody of the mother

    • Personal images of the minor applicant and mother or authorized adult partner
    • PSA birth certificate
    • Passport or valid ID of the mother or authorized adult accompanying person
    • If the applicant is not accompanied by the mother during the application process, the applicant must submit a special authorization letter issued by the mother to designate the minor's accompanying person to assist the application.
