DFA Passport Requirements for First-Time Minor Applicants

Here are the DFA requirements for first-time minors:

• Personal images of the minor applicant and parent or authorized adult companion
• Philippine Statistics Authority (PSA) birth certificate.

• If the child is accompanied by only one parent, a marriage certificate is required.
• If the parents are unmarried, a statement of support and/or consent from the mother is required if the mother is not accompanying the child.
• If the affidavit of support and/or consent was issued abroad, it must be notarized by a Philippine embassy or consulate.
• Passport or valid ID of either parent and one (1) photocopy
• Student ID (if applicable)

The applicant is an illegitimate child under the custody of the mother

• Guardianship Statement.

If the minor applicant has gone through domestic adoption procedures

• Personal profile of the minor applicant and adoptive parents
• PSA birth certificate
• Adopter's valid passport or valid ID card
