Philippine Passport Application Form: New and Updated
One of the basic requirements for applying for a passport, whether for the first time or for renewal, is a duly completed application form.
This form is usually filled out online. When you try to make an appointment, you will be asked to complete an application form. After submitting the form, you will be asked to pay the application fee before they lock your seat. They keep it for 24 hours. Once payment is confirmed, you will be emailed a PDF file containing your appointment schedule and electronically completed application form. All you need is to print the form and an appointment confirmation.

Here are some important notes about the form:
- Print the form on A4 size paper.
- You can create forms digitally or manually. For handwritten forms, be sure to write entries clearly. Use only black or blue ink.
- Please write N/A for fields that do not apply to you.
- A minor is defined as a person under the age of 18. and those who cannot fully take care of themselves.
Bring the completed form along with other required documents to the selected DFA office for an appointment.

Hello! What if I made a mistake in my passport application but I'm past the deadline and they didn't ask me. Is all the information in there important? This is for review. what should I do? A passport renewal form with the name of the Philippine embassy in the host country must be completed, eg page: Philippine Embassy in Qatar; or I can use the standard renewal form template found at: xxxxxx.

Hi Marygrace, is the lost passport still valid? If so, NBI clearance is not required as far as I know. But you will need a police report (in English), a statement of loss and a fine.
